Contractor Checklist
THESE ITEMS MUST BE MET PRIOR TO MORRISON CONSTRUCTION SHOWING UP ON THE JOB. FAX THIS SIGNED FORM TO 209-533-1257.
1)
CONTRACT AND APPENDIX 1-A - Contract and appendix 1-A is signed.

SCHEDULE - Job is scheduled. Other subcontractors are aware of gypsum set-up/pour date(s). We don't want other subcontractors disrupting areas that have been set-up or in areas we are trying to work in.
3)
WATER SUPPLY - Builder has 20 g.p.m. water on site, close to work area, with a ¾ inch hose attachment.
4)
ELECTRIC SUPPLY -Builder has one dedicated 20 AMP 115 VA Circuit.
5)
ENCLOSED - Building is enclosed with roof on.

SUBFLOOR INSPECTION - The subfloor is structurally sound, holes are filled, broom cleaned and contaminant free. The subfloor must be adequate to withstand live and dead loads with a deflection limit of L/360. Any wood weakened or delaminated during construction shall be replaced. Stud wall base plates in doorways and other openings are to be removed unless needed.

RADIANT TUBES are stapled a minimum of 18" o.c. and at the middle of all radius'.

ACCESS -The project is accessible, Ice, mud, materials, other subcontractors, etc., type issues are addressed. There is a minimum setup area of 20 ft x 50 ft outside, accessible with a 10-wheeler semi-truck, unless other arrangements have been made. Morrison Construction Company, Inc. has the rights to refuse the pour if unsafe to proceed and will incur no damages or costs for its decision. Owner will be charged a trip charge if inaccessible.

LEAKAGE, MOISTURE - Morrison Construction Company, Inc. is to be notified of areas on job site, below, or next to areas being poured, containing finished products, expensive equipment, stockpiled materials, etc. that could be damaged by leakage or moisture. Morrison Construction Company, Inc. is not responsible for any damage caused by leakage of product due to voids in wall and/or sub-floor.

MASKING- Protection of finished walls, base boards, wood & log interior siding, cabinets or any other major items is complete by Prime contractor. Doorjambs and minor masking to be done by Morrison Construction Company, Inc.

BLOCKING OF TUBS, ETC. - It is the position of Morrison Construction Company, Inc. that, if tub(s), etc. are in place and Morrison Construction Company, Inc. has contracted to pour under the tub(s), etc, an unobstructed access with full view of the floor area must be provided. The area must be clean and all penetrations must be properly sealed. It is also agreed that any area without unobstructed access and full view has been properly treated by other methods.

MINIMUM THICKNESS for RH is ¾" over wood and over radiant tubing. Any amount less voids warranty.
13)
HEAT - It is the responsibility of the general contractor or the builder to provide temporary heat, if necessary, to maintain a minimum of 60 degrees Fahrenheit during and after the pour in order to cure the floors, and prevent mold.
14)
VENTILATION - Adequate ventilation, fans and even dehumidifiers may be needed to cure floors and prevent mold, and is the responsibility of the Prime contractor and/or owner. Please, note that the USG Levelrock underlayments MAY NEED HEAT, and Dehumidifiers to reach an acceptable moisture level prior to the installation of your floor covering (typically applies in the fall/winter/spring months). Mold may occur if not properly ventilated.

I am fully aware of mold issues and prevention.

ANY OMMISSION, DELETION, ALTERATIONS, OR ADDITIONS by contractor, subcontractor, owner, engineer, architect or their authorized representative other than described in this contract may be adjusted upon completion of this job by actual measurement and the corrected billing at the base price as established herein. Added charges for leveling of jobs will occur if not specified at time of bid. Added charges at a rate of $50 per man per hour will occur if contractor is not ready when we show up.

Please allow two-to-three weeks advance notice for scheduling. Revising your schedule is not advised. Please notify us of scheduling changes as soon as possible. Additional charges apply if cancellation occurs less than 48 hours before a scheduled pour.

CREDIT HAS BEEN APPROVED BY JEFF MORRISON. Credit application has been submitted.
